Ambulation
Knock on the door before entering a room
Say: Good morning
My name is…
I am your nursing assistant for today
Inform the patient what you intend to do: I’m here to walk you 5 steps forward and 5 steps back
Say: Let me wash my hands
Wash your hands if this is the first skill you are performing. For any skill following your first skill you can simply say to the proctor: Handwashing is required.
Return to the patient, say May I check your name tag?
Check their ID and say "Ok (Mr./Ms. followed by the patient’s
last name)
Say: Let me bring my supplies
Bring the transfer belt and put it on the side next to the patient
Say: Let’s get started
Start your procedure in the following sequence:
Align the patient in the chair (Stand behind the patient and pull his/her hips deeper into the chair – Salsa maneuver
)
Stand in front of the patient and put the belt on the patient’s waist, making sure it is not too tight (put two fingers between belt and the patient’s body to check for correct tightness)
Ask the patient to put their hands on your shoulders
Hold the belt on the patient’s sides with four fingers from the bottom and the thumb at the top
Inform the patient that on the count of 3 you’ll stand them up (set up the toe to toe, knee to knee position)
After you stand them, up ask the patient: How are you feeling, are you dizzy?
Stand behind the patient slightly off center. Hold the belt with one hand in the middle of the patient’s back. Hold the belt with the other hand on the patient’s side behind the patient’s arm.
Inform the patient: On my count we move forward with the left (or right) leg first.
Count one,
and the patient takes the first step and the other foot will follow right after to keep balance, count two,
and the patient will take another step. You count and walk the patient until five steps are achieved. Then turn the patient 180 degrees back by saying: Let’s turn to the left (right).
Then walk the patient back to the chair the same way.
When you return to the chair ask the patient to turn to the left (or right) facing you. Make sure the chair is close to the person’s legs. Ask: Do you feel the chair behind you?
If the answer is: No,
ask the patient to take one step back until they confirm that they feel the chair. Always check by yourself if the patient is in a safe position to sit into the chair.
If the answer is: Yes
, sit the patient down on the count of 3 (toe to toe, knee to knee position)
Say: Let me align you
and pull the patient from behind into the chair by adjusting hips in Salsa
maneuver
Say: I will remove the safety belt.
Grab the belt from behind the patient with one hand holding the metal part. Never pull the belt.
Put the belt on the bed.
Offer TV and magazines
Leave the call bell inside the patient’s hand or within reach of the patient’s hand
Say: See you later
Move away from the patient
