November 26, 1999 Dear Mr. Ambrose, RE: Restored Church of God - Installation of Telephone System and Voice Mail purchased from Tri-County Home Nurses Inc. To the best of my recollection the following is a schedule of events pertaining to the installation of Telephone Equipment: Once the purchase price was established, I was put in contact with Steve Thompson, who was handling the coordination of the installation of Telephone Services and Telephone Equipment. Mr. Thompson and I discussed the type of equipment purchased and the type of services needed from GTE. At that time I contacted GTE to review the type of Circuits that had been ordered by Mr. Thompson. The communication between Mr. Thompson and GTE called for PBX circuits, which indicated to GTE that ground start circuits would be required for the equipment, which was incorrect. I changed the order to reflect loop start circuits. This delayed the installation of the service from GTE by at least 8 to 10 working days. When GTE arrived at the location to install the services (One Park Center) it was determined by GTE that there was not adequate entrance cable into the building to install all of the circuits requested. At that time the following lines were installed: 334-2266, 334-0825, 334-9506, 334-3488. Prior to this delay (Amax) had completed the installation of the Telephone and Voice Mail equipment. At this time Amax attempted to connect the circuits provided by GTE to the Telephone System, this was another delay, because of access to the unit occupied by The Restored Church of God. Once the installation was completed, there were continued delays as to the method of interfacing the Voice Mail System and the person responsible for answering, and who that person was and where they were going to sit. There was another delay as for the voice to be recorded on the greeting of the Voice Mail System. By the time these parameters were established, there was a change in personnel, which required more changes to the Voice Mail and the location of the person answering the Attendant Telephone.
