Robert E. Adap owns a Honda City car with plate number PIJ 852. On July 6, 2012 while driving on the National Road in Caranglan, Nueva Ecija with his office aid Guillermo Balangue, a stone pebble hit the right side of the front bumper and brushed off the right headlight. This caused damage to the front bumper, which incurred a dent, and the headlight, which cracked. Robert E. Adap executed this affidavit to support his claim for indemnity for the damages to his car from this incident.

AFFIDAVIT
I, ROBERT E. ADAP of legal age, married and a resident of Macanaya, Aparri, Cagayan, under oath, hereby depose and say that : 1. I am the registered of a car, a Honda City with Plate No. PIJ 852; 2. In a trip to Manila on July 6, 2012, at on or about 8:00 oclock in the evening while I was driving the said car with my office aid, Guillermo Balangue along the expanse of the National Road at Caranglan, Nueva Ecija which is currently under repair, a stone pebble hit the right side of the front bumper thereof and likewise brushed off the right head light of the car as it flew off the air; 3. As result of the said accident, the front bumper and head light of my car both on the right side have been damaged, that is, the bumper incurred a dent and the head light had cracked. 4. I am executing this affidavit to support my claim for indemnity for the damaged into my car and that I attest to the truthfulness of the foregoing statements. IN WITNESS WHEREOF, I hereunto set my hand this 18th day of July 2012 at Tuguegarao City.
