This agreement appoints Fidelity Research & Recovery, LLC as the exclusive agent of Ryan Marron to recover $2,257.10 owed by a government agency. It grants FR&R limited power of attorney to obtain necessary proofs, sign and file the refund application, receive and deposit any refund check, and deduct their fee before disbursing the remainder to the client. AGREEMENT

AGREEMENT made this ________ day of ____________, 201_ by and between FIDELITY RESEARCH & RECOVERY, LLC of 200 W. Ohio St., Suite 200, Chicago, IL 60654 (hereafter referred to as FR&R, LLC) and Ryan Marron of Address (hereafter referred to as Client). Client and FR&R, LLC constitute the sole parties to this agreement. Client hereby designates and appoints FR&R, LLC as its exclusive agent to recover $2,257.10 owed to Client from a government agency. Client grants FR&R, LLC a limited power of attorney to do the following as it relates to this recovery: 1. 2. 3. 4. Obtain all necessary proofs required for the refund application. Sign and file the refund application on behalf of Client. Receive and deposit the refund check. Deduct FR&R, LLCs fee before final disbursement.

Client agrees to pay FR&R, LLC a fee of 33.33 percent (thirty three and one-third percent) of the sum actually received from the government agency making the refund. This fee shall constitute the sole amount of money to which FR&R, LLC is entitled and Client shall not be responsible for any other fees, costs or expenses borne by FR&R, LLC. If FR&R, LLC cannot collect any money on behalf of Client, Client owes FR&R, LLC nothing. FR&R, LLC makes no representations or warranties concerning the successful recovery that may be made on behalf of Client. All statements of FR&R, LLC on these matters are statements of opinions only. Client agrees to cooperate with FR&R, LLC in providing all documents pertaining to said recovery attempt that are within clients reasonable ability to obtain, and signatures as may be required. Should the terms of this agreement be broken, FR&R, LLC retains the right to seek legal remedies including all costs incurred, and all reasonable attorneys fees relating to such.
