Child has brown hair, green eyes, normal appearance. Normal skin color, muscle turgor, and tooth development. C. Low income family. Interacts well with his brothers and sister. Spends about 7 hours a day 5 days a week at school. Health history A. Male, 5 years old B. Birth date: August 15, 2007 Birth weight: 6 pounds 14 oz Birth length: 19 inches C. Current weight of 38 pounds and 42 inches tall D. Aaron has 3 siblings the oldest being 7 years old, then 3 years old, and a 1 year old sister. E. The mother has experienced recent weight gain. She has given birth to 4 children and is currently pregnant. F. He has had small head deformations at birth that were corrected by a medical helmet. He has had bronchitis. G. No H. Aaron has had a cavity at age 4 but has had good dental records since. I. No known allergies Developmental skills assessment A. Aaron completed all task accurately and proficiently. He was able to prepare cereal, brush his teeth, copy, draw a person, define 7 words, identify opposites, count blocks, identify adjectives, balance on each foot for 6 seconds, and walk heel-to-toe. He was a little distracted during some of the test but not outside any of what would be considered normal for his age. B. There were no cautions, delays, or no-opportunity. The test showed no signs of underdevelopment. A. Aarons calorie distribution is 13% calories from protein, 35% from fat, and 52% carbohydrates. These macronutrient distributions are acceptable. Aaron ate 8 ounce equivalents of grains, 0 cup equivalents of vegetables, 0 cup equivalents of fruits, 2 and 1/4th cup equivalents of dairy, less than 1 ounce equivalent for protein foods. He over consumed grains by about 3 oz and under consumed vegetables, fruits, and protein foods. Aaron is getting sufficient iron and calcium. He is under consuming potassium, vitamin A, vitamin C, and vitamin D. Aaron was born at the 50th percentile for weight for length meaning he had experienced normal growth up to the point of birth. Aarons current stature for age and weight for age are both at the 10th percentile showing that he is small for his age. His weight for stature is between 10th and 25th percentile. B. He does not have any food jags, he dislikes a good number of foods but this does not stop him from feeding normally at meal times. The mother offers rewards to Aaron for finishing his food and suggest punishment for not finishing. C. The family has dinner ever night together at the table. The schedule is not set in stone and so fluctuations of up to two hours sometimes occur in meal schedule patterns. D. The child takes a mineral vitamin supplement once a day about three times a week. E. The family does not use any type of food assistance programs. V.
Nutritional care plan and implementation

B. Aaron is not eating enough vegetables and fruits. He should be eating a least 1 cups of vegetables and 1 cups of fruit.The majority of his calories are empty calories. He should eat more meat such as fish which would provide him with essential fatty acids and a good source of protein. If he could eat 3 to 5 more ounces of protein foods. He would do better if he could eat about 200 more calories from nutrient dense foods a day. This would help him achieve more growth and achieve the goal of reaching the 50th percentile for stature for age, weight for age, and weight for stature. C. I would suggest that Aaron should eat more vegetables and fruits. This can be accomplished by offering him the vegetables and fruits at mealtimes as well as having the parents increase their consumption of vegetables and fruits. I will also suggest adding at least one more snack during the day and that the snack be at least 200 calories worth. This will increase Aarons total calorie intake to an optimal level.

I will also suggest certain practices to the mother and father to make meal times more regular. This can be accomplished by setting specific times for snacks and meals in which the family can eat together. I will suggest that it would be best not to stray from the schedule. I would also suggest letting the child assume more responsibility for his eating habits. This can be done by not placing rewards or punishments with foods or eating habits.
