Physical Medicine and Rehabilitation Departement: Manual Muscle Testing

The document discusses manual muscle testing (MMT) and describes the grading system used. It explains that MMT involves having a patient contract their muscle against resistance provided manually by the examiner. The grading system ranges from 0 to 5, where 5 is normal muscle strength against maximum resistance, 4 is good strength against some resistance, 3 is fair strength to move through full range of motion against gravity, 2 is poor strength to move part of range of motion against gravity. Trace (1) and zero (0) indicate minimal or no muscle contraction detectable through palpation. The document also briefly outlines different types of strengthening exercises - isometric, isotonic, and isokinetic - as well as stretching and endurance exercises.

INTRODUCTION

PHYSICAL MEDICINE AND REHABILITATION DEPARTEMENT

Manual muscle testing

FACULTY OF MEDICINE BRAWIJAYA UNIVERSITY Dr. SAIFUL ANWAR GENERAL HOSPITAL

HOW TO EXAMINE MANUAL MUSCLE TESTING ( MMT )?

THE GRADING SYSTEM BY Dr. R W LOVETT : 1. NORMAL = 5 2. GOOD = 4 3. FAIR = 3 4. POOR = 2 5. TRACE = 1 6. ZERO = 0

NORMAL = 5
THE PATIENT AGAINST AMOUNT OF RESISTANCE THAT CAN GIVEN MANUALLY BY EXAMINER TO CONTRACTED MUSCLE OR MUSCLE GROUP

GOOD = 4
THE PATIENT AGAINST THE SOME OF RESISTANCE THAT CAN GIVEN MANUALLY BY EXAMINER TO CONTRACTED MUSCLE OR MUSCLE GROUP

FAIR = 3
THE ABILITY OF THE MUSCLE OR MUSCLE GROUP TO MOVE A COMPLETE RANGE OF MOTION WITH THE AGAINST GRAVITY

POOR = 2
THE ABILITY OF THE MUSCLE OR MUSCLE GROUP TO MOVE A PART THROUGH A COMPLETE RANGE OF MOTION WITH THE AGAINST GRAVITY OR THE ABILITY OF THE MUSCLE OR MUSCLE GROUP TO MOVE A COMPLETE RANGE OF MOTION OF THE HORIZONTAL PLANE

TRACE = 1
ANY OF A MUSCLE CONTRACTION IS DETERMINED BY PALPATION THE MUSCLE BULK

ZERO = 0
AN ABSENCE OF A MUSCLE CONTRACTION IS DETERMINED BY PALPATION THE MUSCLE BULK

3. TYPE OF STRENGTHENING EXERCISE :

a. ISOMETRIC EXERCISE / STATIC EXERCISE * ANY MUSCLE CONTRACTION. * NO JOINT MOTIONS * MUSCLE CONTRACTION IN 6 SECOND. * PRECAUTION IN HYPERTENTION & CORONARY HEART DISEASE PATIENT.

b. ISOTONIC EXERCISE ANY MUSCLE CONTRACTION WITH JOINT MOTION. 2 TYPE :

CONCENTRIC CONTRACTION : SHORTHENING ECCENTRIC CONTRACTION : LENGTHENING

c. ISOKINETIC EXERCISE ( COMBINE OF ISOMETRIC & ISOTONIC )

USED A TOOL CONTANTLY SPEED MORE SAFETY FOR HYPERTENTION & CHD PATIENT

3. STRETCHING EXERCISE :
FOR A STIFFNESS OR CONTRACTURE OF JOINT. THROUGH A PAIN POINT. SHOULD NOT BE EXERTED

4. ENDURANCE EXERCISE
THIS EXERCISE INCLUDE STRENGTHENING & ENDURANCE. IN STRENGTHENING EXERCISE : OPTIMAL RESISTANCE & LOW FREQUENCY IN ENDURANCE EXERCISE : LOW RESISTANCE , HIGH FREQUENCY & LONG DURATION. CONDITIONING : IN HEALTHY PEOPLE FOR INCREASING ENDURANCE RECONDITIONING : IN ILLNESS PEOPLE FOR ENDURANCE RECOVERY
