FOR GRADUATE PROGRAMS January 10, 2014 (Friday) - Deadline for filing of application forms with the required documents for First Semester AY2014-2015 March 19, 2014 - Doctoral Admission Test in Education (DATE) March 20 & 21, 2014 - Masters Admission Test in Education (MATE) Documents needed to support your application: 1. Original and two (2) photcopies of your Official Transcript of Records (undergraduate and graduate work, if any) with any of the following remarks: FOR FURTHER STUDIES, ISSUED UPON REQUEST, or FOR BOARD EXAMINATION. (All other remarks are not accepted by the University) 2. Accomplished application forms (2 sets) download here 3. Official receipt of payment of a non-refundable application fee of Php700.00 (for Filipino applicants) or US$30.00 (for Foreign applicant) paid to the UP Cashier, UP Diliman, Quezon City. Trust Fund Account No. 9786600-499-450. 4. Two (2) reference reports from former professor and one from current/immediately previous employer. download here 5. Five (5) copies of 2x2 recently taken picture (not computer scanned nor photocopied) 6. Original plus one (1) photocopy of NSO-issued Birth Certificate. 7. Original plus one (1) photocopy of NSO-issued Marriage Contract (for married female applicants) 8. Self-addressed, stamped envelope (SASE) and one long folder 9. Permit to study from employer (if currently employed) 10. Official copy of the result of your Test of English as a Foreign Language (TOEFL) examination. (For foreign applicants whose native language or whose medium of instruction is not English).
