The student nurses searched for a suitable family that fit the criteria. After establishing professional relationship and obtaining consent, physical assessment was performed on every member of the family that was present at the time. They also conducted an interview to obtain the necessary data like the familys health history.

2nd Home Visit (November 07, 2013)

For the second home visit, the student nurses went to the Reyes Familys home. Physical assessment was once again conducted. Interview was done to acquire the necessary data that was missing from the interview the day before. Interventions were done by helping the father clean the house and assisting in the grooming of his children. Health teachings were also provided by the student nurses based on the observations, data gathered and problems identified.

3rd Home Visit (November 08, 2013)

For the third and last home visit, the student nurses conducted physical assessment and interview on the family again to assess for any significant changes. Further health teachings were given before they left.
