Data Objective: -

Nursing Diagnosis Risk of infection related to destruction of tissue and increased environmental exposure

Rationale Contamination of a wound surface with microorganism thus these colonization has a complete new cells for oxygen and nutrition and because their byproducts can interfere with a healthy surface condition that leads to infection

Objectives After an hour of intervention, patient will be able to: -Take in and identify proper foods needed to enhance milk production -Be aware of medications, its importance and effects

Nursing Interventions -Perform perineal care

Rationale -To promote cleanliness to the perineal area
