Scanning Instructions (Instructions may not be applicable for all scanners)
1. Place the sheet you need to scan in the top/front slot of the printer/scanner device. The Scan window opens on the administration workstation screen. Click the Scan button. 2. Set the Select where scan should go option to Image File. 3. Click Scan. 4. Set the filename to the candidates last name, the current date (MMDDYY) and the candidates Registration ID, as in larsen1006031234567. 5. In the Save as Type field, select .PNG 6. Click Save.

Scanned forms must be attached to the Backup Biometric incident within 24 hours. If this is not possible, contact Channel Quality for a fax number. Channel Quality will be able to scan a faxed copy of this form and attach it to the incident.
