The patient has a fair prognosis based on the assessment. While he has some difficulty following his diet and medication regimen consistently, he is generally compliant with treatment and has family support. He is able to perform basic activities of daily living but his illness has impacted his normal functioning. The patient's environment is suitable and he has an adequate support system.

PROGNOSIS Criteria GOOD FAIR POOR Justification According to Mr.

LP he cannot function normally like before because of his confinement and Compliance to treatment regimen. condition but still he wants to take all the medication that the doctor prescribed him, in order for him to cure his disease. There is an adequate space in his Environment house and well ventilated. In the afternoon after lunch he likes to watch TV. Ability to perform ADLs Pt.able to perform ADLs when his in the hospital like ambulation, deep breathing exercises . The pt has a very supportive family Support system especially his wife that assisted him through his hospitalization days. It depends on the pt. if he is compliance to the treatment. Sometimes he follows his medication and sometimes she is not followed. Before he was hospitalized he likes to eat fruits and vegetables and rarely eat meat. But after hospitalization he can eat whatever he likes but verbalized that he loses his appetite.

Diet as tolerated was his diet but due to his condition he loses to eat more and has lack of appetite.

Score: POOR: 1X 1 = 1 FAIR: 3X2=6

GOOD: 2 X 3 = 6 Total : 13 / 6 = 2.1 (Fair Prognosis) Ranges: POOR: 1.0 1.6 FAIR: 1.7 2.3 GOOD: 2.4- 3.0 General Prognosis: We were able to come up with fair prognosis based on the management care given. The client wasnt able to follow her diet but cooperated in taking medications. She has also supportive families that extend the care to the patient. From our given prognosis, we can say that it is a fair prognosis to our client.
