Domperidone

Domperidone is a dopamine antagonist used as an anti-emetic and to treat nausea, vomiting, and vertigo. It works by blocking dopamine receptors in the chemo-emetic trigger zone. Adults take 10-20 mg doses 3-4 times daily before meals and bedtime depending on the condition. Domperidone is contraindicated in those with hypersensitivity, prolactin-releasing pituitary tumors, or where increased gastric motility could be harmful. Potential adverse effects include allergic reactions, abdominal cramps, dystonic reactions, increased prolactin levels, and hypertensive crises in rare cases. Nurses should monitor patients for extrapyramidal effects and hypotension.

GENERIC NAME: Domperidone DRUG CLASS: Dopamine Antagonist, Anti-emetic and anti-vertigo PHARMACODYNAMICS: Domperidone is a dopamine-receptor blocking agent.

Its action on the dopamine-receptors in the chemo-emetic trigger zone produces an anti-emetic effect. DOSAGE: Adults: Two tablets (20 mg) 3 to 4 times per day, 15 to 30 minutes before meals and, if necessary, before retiring. Chronic conditions (mainly dyspepsia) Adults: One tablet (10 mg) taken 3 times per day, 15 to 30 minutes before meals and, if necessary, before retiring. The dosage may be doubled INDICATIONS: Acute conditions (mainly nausea, vomiting, hiccup) CONTRAINDICATIONS: -Known hypersensitivity to domperidone or any of the excipients. -Prolactin-releasing pituitary tumour (prolactinoma.) -Domperidone should not be used when stimulation of gastric motility could be harmful: gastrointestinal haemorrhage, mechanical obstruction or perforation. ADVERSE EFFECTS: Allergic reactions, such as rash or urticaria, have been reported. Abdominal cramps have been reported. Dystonic reactions (extrapyramidal phenomena) may occur. Reversible raised serum prolactin levels have been observed which may lead to galactorrhoea and gynaecomastia. Hypertensive crises in patients with phaeochromocytoma may occur with administration of domperidone. Where the blood brain barrier is not fully developed (mainly in young babies) or isimpaired, the possible occurrence of neurological side-effects cannot be totallyexclude NURSING RESPONSIBILITIES: 1. Assess for extra-pyramidal effects such as jerking and tongue protrusion. 2. Check for hypotension
