Cardiovascular Therapeutic Management

This document discusses various therapeutic management options for cardiovascular alterations including pacemakers, intraaortic balloon pumps, percutaneous coronary interventions such as angioplasty and stenting, fibrinolytic therapy, and cardiac surgery. It provides details on how pacemakers and intraaortic balloon pumps work and their indications. It also briefly outlines three common percutaneous coronary interventions and the general indication for fibrinolytic therapy.

Implantable Cardioverter Defibrillator
Percutaneous Transluminal Coronary
Angioplasty
Thrombolytic Therapy
Intraaortic Balloon Pump
Pacemakers
An electrical devise that produces and
delivers and electrical stimulus to the
myocardium when the hearts intrinsic
electrical system in unable to effectively
generate a rate adequate to support cardiac
output.
Indications for Temporary
Pacemakers

Bradydysrhythmias
Tachydysrhymias
Sick sinus syndrome
Support after cardiac surgery
Diagnostic studies
Pacemaker Rhythms
Failure to Pace
Failure to Sense
Intraaortic Balloon Counter
Pulsation (IABP)
The most widely used mechanical assist
device to support failing circulation.
A catheter that is inserted into the femoral
artery and threaded up into the aorta.
A balloon surrounds the catheter and is
timed to inflate during diastole and deflate
just prior to systole.

Indications for IABP
Left ventricular failure
Unstable angina refractory to medications
Complications of AMI
Cardiogenic shock
Papillary muscle dysfunction
Ventricular septal defect
Refractory ventricular dysrhythmias

Intraaortic Balloon Counter Pulsation
(IABP)

Diastolic Augmentation
Inflation during diastole
pushes blood back into the
coronary arteries, thus
increasing myocardial
oxygen supply.

Afterload Reduction
Deflation just prior to
systole reduces left
ventricular afterload, thus
reducing myocardial
workload and oxygen
demand.

Complications of IABP
Limb ischemia
Aortic Dissection
Embolism
Thrombocytopenia
Bleeding

Gas embolism
Infection
Renal failure
Compartment
Syndrome

Three Percutaneous Coronary
Interventions (PCIs)
1. PTCA:
Percutaneous
Transluminal
Coronary Angioplasty

2. PTCA + stent
placement

3. Atherectomy:
grinds away
the plaque
Fibrinolytic Therapy
Breaks up the fibrin network that binds the
clots together.
Indications: ST segment elevation > 1mm in
2 or more continuous leads.
Administration must take place within 12
hours of onset of symptoms.
