I agree to observe and obey all rules and further agree to follow any oral instructions or directions given by instructors of Karate For Christ.
I recognize that that there are certain inherent risks associated with the above described activity and I assume full responsibility for personal
injury to myself and further release and discharge Karate For Christ for injury, whether caused by the fault of myself, Karate For Christ, or
other third parties.
I agree to indemnify and defend Karate For Christ against all claims, causes of action, damages, judgments, costs or expenses, including
attorney fees and other litigation costs, which may in any way arise from my use of or presence upon the facilities of Karate For Christ.
I agree to pay for all damages to the facility of Karate For Christ caused by my negligent, reckless, or willful actions.
I have read this document and understand it. I further understand that by signing this release, I voluntary surrender certain legal rights.
