I am informing you of a missed appointment regarding James P. Henson.
Mr. Henson failed to accommodate his appointment on September 20, 2014 at 4:00 p.m. If you
so desire, you may telephone our office to schedule another appointment. However, if you prefer
to have another physician attend him, be sure to do so without delay.
If you chose to seek another physician we will be sure to transfer any medical knowledge
regarding your case to the physician of your choice at your written request.
I trust that you will understand that our purpose in writing this letter is out of concern for his
health and well-being.
