Risk Assessment Recording Form

(1) The risk assessment form identifies potential hazards in a classroom such as electrocution from loose wires, slip hazards from liquid spills, and risks of falling or a computer catching fire. (2) Those at risk are identified as students and tutors who could be injured by touching loose wires, slipping on spills, tripping and falling, or being burned.
