Dear Alena Easter,
This will serve as your written warning concerning your absence without prior notification. Specifically, you were absent from your shift with Dr. Srinivas on 2/16/15 from 9pm-6am without any forewarning to either him or myself. There was also no response to my email the next day inquiring about the absence. Record of this offense is documented and readily available for you to review on the ADP system. Your failure to comply with our Company expectations of showing up to work negatively impacts the start of the physician shift, often times being the most critical hour in terms of patient volume. In addition, it negatively impacts the level of customer service that we provide to our clients. You must show immediate improvement and sustain resolution of your attendance. Failure to do so will result in further disciplinary action; including termination of employment. I would like to see you succeed; however, your job is now in jeopardy. Respectfully,
