Classroom Rules Contract

The student agrees to follow the five classroom rules of being respectful, kind, responsible, a good listener, and prepared. If the student receives three warnings in a day, the teacher will sign their behavior log and provide a reason. If the student receives three signatures in a week, the teacher will contact their parent. At the end of the month, if the behavior log has five total signatures, the teacher will discuss the behavior with the student and parent. The student and parent sign to acknowledge understanding and agreeing to the rules and consequences.

Contract for Class Rules

4/3/15

I, (students name), understand The Five Bs class rules. The Five Bs are: Be Respectful, Be Kind, Be
Responsible, Be a Good Listener, and Be Prepared. I know what is expected in Miss Dorstens class and I
know how I should behave. I understand what the behavior log is and I will staple in the front of my agenda
every month. I know that if I receive THREE warnings in ONE day, the teacher will sign my behavior log for
that day and give a reason. I understand that if I receive THREE signatures in ONE week Miss Dorsten will
contact my parent/guardian and inform them of my misbehavior. I understand at the end of the month Miss
Dorsten will review my behavior log and if I have FIVE signatures for the whole month Miss Dorsten will
discuss my behavior with me and have a parent-teacher conference.

In Miss Dorstens class I will follow The Five Bs and if I dont I know what the consequences are.
