Dear Sir/Madam:
Greetings in the name of St. La Salle! It has been 3 days after the required date of
submission of the documents that we requested from your office. However, you failed to
submit all, if not, a number of papers. Having the power, authority and duty to examine
and audit all financial accounts of every USG unit, we are demanding your office to
provide us the following documents in order to complete our task, namely:
1 FRB(Hard Copy or Soft Copy)
2 Income Statement (Hard Copy or Soft Copy)
3 Statement of Government Unit Responsibility
For OTREAS only,
4 End Term Balance Report
Please hand it to us the earliest time possible. The deadline for your compliance will be
on September 26, 2014, Wednesday but the sooner, the better. Kindly contact
Jessamine Alvaro at 09173274992 and [email protected]. Failure to
comply with the requirement will compel us to issue a disclaimer of opinion, which is
tantamount to undesirable.9
