AFFIDAVIT OF LOSS
I, DREVELYN P. MATIAS, of legal age, single, Filipino citizen, with
residence and postal address at Manaba, Bulalacao, Mankayan, Benguet,
after having been duly sworn to in accordance with law do hereby depose
and state that:
1 I am a holder of a RELEASED/TERMINATION LETTER issued by Hong
Kong Government;
2 Said Released/Termination Letter was issued to me when I asked the
Hong Kong Government to terminate me. However, said
Released/Termination Letter was lost/missing since December 10,
2013. The folder containing the said Released/Termination Letter
was misplaced, either in our house or in my boarding house;
3 I exerted diligent efforts to locate the said RELEASED/TERMINATION
LETTER but the same could no longer be found and hence
considered forever lost;
4 Said RELEASED/TERMINATION LETTER was never given nor pledged
to anyone for any purpose whatsoever, nor was it taken or
confiscated by the authorities for any violation of the law;
5 I am executing this affidavit to attest to the veracity and
truthfulness of my foregoing statements and for other lawful intents
and purposes it may serve.

February, 2015 at Baguio City, Philippines.
