Change of decoder ownership form

Please complete the following form to change the ownership of a decoder. The completed form and other
required documents can be dropped off at a MultiChoice Service Centre or Agency, sent via email to
[email protected] or faxed to 011 577 4901.
The following documents are needed to complete the request for change of ownership:

Certified copies of the IDs of previous and new owner

If bought from a second hand retailer, proof of purchase

If previous owner is deceased, an affidavit from the deceased estate or certified copy of the death
certificate

If previous owner is not known to new owner, an affidavit
