AFFIDAVIT OF DISCREPANCY
I, KIRT RUSSEL CATEQUISTA PECONADA, of legal age, single, Filipino
and a resident of Angie Subdivision, Kidapawan City, Philippines, upon oath,
hereby depose and say:
1. That I am the same person whose personal circumstances
appear above;
2. That in my Certificate of Live Birth, my last name was registered
therein as PECONADA which is the TRUE and CORRECT
spelling;
3.

That however, in the same document, the last name of my


father was registered as PECONDA7 instead of PECONADA;

4. That the discrepancies mentioned above were made


inadvertently and were not intentionally made to defraud nor
cause confusion as to my true identity; and
5. That I am executing this affidavit to apprise the proper
authorities concerned and for the purpose of supporting my
licensure exam for Accountancy before the Office of the
Professional regulation Commission (PRC) and for whatever
legal purposes this may serve best.
