GOOGLE FORMS TUTORIAL

Step 1: Click on the Google apps symbol and choose the Google Drive icon.
Step 2: Log into your Google Account.

Step 3: Click on Create and select Form

Step 4: Add a title, description, and question type for your Form.

Step 5: Click Insert to choose from nine different question types.
Step 6: Choose layout to insert an image, video, page break section header.

Question Type Descriptions


Responders provide short answers, usually just a few words.

Paragraph Text

Responders provide longer answers paragraph(s).

Multiple Choice

Responders choose one option among several.

Checkboxes

Responders select one as many options as they would like.

Choose from a list

Responders select an option from the dropdown menu.

Scale

Responders rank an item along a scale example from 1 to 10.

Grid

Responders select a point from a two dimension grid.

Date

Responders use a calendar to enter a date.

Time

responders select a time (time of day or duration)

Step 7: Choose features. Edit questions, Change theme, and View live form.

Change theme Choose from a variety of themes or customize one of your own.
View Live Form View the form as visitor will view it.
Edit Questions continue to add/delete/change questions.

Step 8: This screen will


appear when you click on
Send Form.

Step 9: Forward the link to


your participants. Check
the Short Url box to
generate a shortened link.

Step 10: Submissions to your form will be populated into a Google Spreadsheet. To
access the results of your form, open the form, click Edit, then choose View
responses.

Note: Results will populate


in a spreadsheet. Click Data
to sort responses.

Note: Click Form

Show Summary of responses will generate


display graphics and analysis of the data.
