Dear Sir/ Madam,
We hereby accept your bid for the above-stated Contract and therefore, award the Contract to you, as the bidder
with the Lowest Calculated Responsive Bid at a Total Contract Price equivalent to Three Million One Hundred
Forty One Thousand Nine Hundred Ninety Two Pesos & 86/100 (P 3,141,992.86 ).
You are therefore required, within ten (10) days from your receipt of this Notice of Award, to submit to us (a) this
Notice with your signed conforme thereon, (b) the required Performance Security, (c) the required Credit Line,
and (d) the PERT/CPM diagram and Bar Chart with S-curve, (e) Construction Methods, (f) Construction Safety and
Health Program approved by the DOLE, all of which will form part of the Contract Agreement in accordance with
Bidding Documents. You are also required, within the said ten (10) days, to formally sign the Contract Agreement.
Your performance security shall remain valid until issuance by this office of the final Certificate of Acceptance.
Your failure to comply with these requirements shall constitute a sufficient ground for the cancellation of this award
and the forfeiture of your Bid Security.

were found eligible;
WHEREAS, the Approved Budget for the Contract (ABC) is in the amount of A. P 750,000.00, B. P 300,000.00,
C. P 300,000.00, D. P 600,000.00, E. P 1,200,000.00;
WHEREAS, the BAC received five (5) bids for the contract on July 19, 2012 and passed the preliminary
examination;
