Issue Report: Discrepancy of Overtime Hours Rendered
Details of Issue:
1. Signed O.T. form (during breaktime of OT hours) for 4hours accessory attachment from 14:00
to 18:00.
2. On 4:20pm at sub-assy process, task assigned to me was done and my Senior Team Leader
tried to look for other task at Crimping process.
3. At the Crimping process, the task involved is not appropriate or suited for my level and it will be
risky for my safety.
4. The Senior Team Leader assigned me the 5S routine to any area that needs my service up to
5:00pm. Then my Senior Team Leader decided to send me home that there are no more tasks
available for my service during that time.
Problem:

Action Taken:

The signed OT Form is good for 4 hours wherein


the OT hours rendered is only 3 hours. I am
already at my residence when realized to change
the form I signed.

1. The following day 17:, I informed the


immediate superior/ Senior Team Leader
about the issue.
2. I asked my Senior Team Leader, on how to
correct the discrepancy.
