Donburn Wint Private Lessons Contract

Contract for piano lesson


Payment
Tuition is a flat monthly fee of $240.00 a month. Individual lessons have
no exact value. Tuition is due no later than the first lesson of the
month. Services may be refused if payment has not been made on
time. Payment should be made in the form of cash or check made out
to Donburn Wint. Payment may not be broken into smaller amounts
unless prior arrangements have been made.

Cancellations by student.
If you need to cancel a lesson for any reason it must be done as soon
as there is a known conflict. A make up will be scheduled if possible
and if it meets the criteria of acceptable cancellations. A total of 1
lesson may be made up a month, and a total of 3 lessons will be made
up within the span of the year.

Cancellations by teacher.
If I have to cancel a lesson for any reason I will do so as soon as the
conflict is known. A make up will be scheduled.

Cancellations by act of nature.


If there is an uncontrollable and unforeseen event that prevents any
lesson(s) from taking place, the teacher will contact the student
immediately to inform you of any lesson that will not be held. A makeup will be scheduled. Examples of such events would be inclement
weather, power outages or local/national emergencies.

Make-Ups
Make-ups WILL be granted for the following acceptable cancellations:
. Sickness (Notify me as soon as possible by phone)
. Educational school event such a field trips or science fairs.
Must have at least two (2) weeks
notice.
. Band/Chorus Concerts that the student is directly involved in.
Must have at least two (2) weeks
notice.
. Special Religious Events that the student is directly involved in
(such as a First Communion)

Make ups can NOT be granted for the following unacceptable


cancellations:
. Sporting Events
. Birthday Parties/Social Events
. Family Vacations
. Forgetting a lesson

. Inadequate notification of a conflict (ie less than two weeks


notice for band concert)

Withdrawal from lessons


There is a required minimum of one (1) months notice of intent to
withdraw. If a student withdraws from lesson with less than one (1)
months notice this may result in a charge for all or part of the next
months lesson fee.

Holidays - The following days there are no lessons. No make-ups


are scheduled for these days:
January 1st
July 4th
Wed, Thu and Fri of Thanksgiving week.
December 24th through December 31st.

By signing this contract I acknowledge that I have


read and understand the terms and conditions of
this contract. I have a copy of this information for
future reference.
