Community Service Form Adopted 8 2011

Desiree Leal volunteered 6 hours at Sunset Elementary helping a 3rd grade teacher set up his classroom before the school year and assisting at the school's Fall Carnival. This gave her experience in classroom setup and allowed her to participate in the Sunset community by interacting with students, parents, and faculty. Desiree also volunteered 6 hours helping with setup, organization, and cleanup for an annual MDA soccer tournament that raises funds to support families affected by muscular dystrophy and research for a cure. The tournament brought the community together for an important cause.

Type of Community Service: Volunteer

Number of Hours: 6 Date(s): August 9-10, 2013 & October 17, 2014
(6 hours minimum per semester)

Explain your involvement and how you and the community benefited from this service:
I volunteered my time at Sunset Elementary helping Mr. Contreras a 3rd grade teacher set up his classroom before the
school year started. It was a great experience because I was able to get the experience of setting up a classroom for a
new in coming group. By doing this I now see the time and dedication that teachers have to put into their room in order to
make learning accessible. The second community service I did took place during Sunsets Fall Carnival. At the carnival I
helped Mr. Contreras set up his game and ran the game for the duration of the carnival. This was a fun experience
because I was able to be apart of the Sunset community. This was a great way to meet the parents of students and also
the faculty of Sunset Elementary.

California Mini-Corps Community Service Form

Mini Corps Tutor: Desiree Leal

Agency/Organization: Muscular Dystrophy Association & Alpha Pi Sigma

Contact Person: Natalie Alvarez

Phone # : (559)740-9572

Type of Community Service: Soccer Tournament

Number of Hours: 6
Date(s): October 18, 2014
(6 hours minimum per semester)

Explain your involvement and how you and the community benefited from this service:
The annual MDA Co-Ed soccer tournament is a event which helps raise funds for children who suffer from Muscular
Dystrophy. Muscular dystrophy is a muscle disorder that consists of the weakening and tissue loss of the muscles. Over
time, the condition gets worse and those suffering form it lose the ability to walk. For the event, I helped with the set up
and organization of the tournament as well as the cleaning of the location afterwards. This event benefits the community
because all of the money raised helps support needy families who can t afford to pay doctor sessions. Also, the money
helps fund research projects for a cure as well as raise money for the MDA summer camp. This is a great community
service event in which people get together to raise money for those who need help as well as fight for a cure.
