which was impounded and taken into custody by the Philippine National
Police Pasig City, and in connection therewith, I hereby declare under oath
that:
1. 123 Company is the registered owner of the aforesaid vehicle, as evidenced
by the Certificate of Registration and the Official Receipt, copies of which
are attached hereto as Annexes A and A-1, respectively;
2. I have been and am duly authorized to process and secure the release into
my custody of the above-described vehicle, as evidenced by the Secretarys
Certificate dated __ January 2015 issued by the Corporate Secretary of 123
Company, a copy of which is attached hereto as Annex B;
3. The said vehicle is not an instrument or tool nor the proceeds in the
commission of any offense;

4. I undertake to produce the said vehicle before your Office or the proper
court whenever so required;
5. I have caused the said vehicle to be photographed and certified by the police
custodian as accurately representing the same, copies of the pictures taken
are attached as Annexes C and C-1, respectively;
6. I hereby affirm this statement I executed this __ day of January 2015 before
the Office of the Prosecutor and hereby bind myself under penalty of law to
appear and testify thereon in court whenever so required.
