Transfer of Stock Certificates: (See Paragraph 16)

This document is a form for transferring stock certificates. It assigns and transfers interest or shares in a stock certificate from a transferor to a transferee. It requests that upon registration of the transferee as the new holder, the stock certificate be renewed or converted into the transferee's name for the portion transferred. The transferor requests that for any portion not transferred, the certificate be renewed in their name. Both transferor and transferee sign in the presence of a witness to complete the transfer.
