General Instructions
1. This form should be accomplished for each project and the whole program that generates
income.
2. Submit 6 copies of report of income generated together with the Quarterly Financial Report.
3. Upon completion of the program/ project this shall also accompany the terminal technical
and financial report in which case report of income generated for the whole duration should
be made.
