*Fundraising-each family involved in ball is required to sell 1 case of chocolates. Once sold, you keep the $60.

*Tenatively Baseball Practice Mon/Wed and Softball Practice Tues/Thurs.

CUT OFF FOR REGISTRATION IS APRIL 8, 2016


The above player has my permission to participate in the above program. I do hereby release, absolve, indemnify
and save harmless Bentley Minor Ball, their board members, supervisors and coaches and any and all of them, from
claim which I or the said player may have as a result of his/her participation. I do assume all the risks and hazards incidental
to this activity and hereby waive all claims against the above organization.
