Description of Learners: 34 yr. old female
Instructional Objective: Must use a verb that aligns with Blooms Revised Taxonomy and has
at least two parts, learning and behavior.
After learning how to use Survey Monkey, the student will design their own survey that
includes 8-12 questions focusing on a specific topic.

Anticipatory Set: Describe how you will engage students and connect to their background
knowledge. (1-2 mins)
Ask the question: Have you ever wanted to ask parents or students a few questions, but
didnt want to hold a long discussion? Think about a time when you wondered if parents
thought a certain unit lesson influenced their child or a field trip was inspiring. Have
you asked students how their collaboration groups were going? Only to get a long
discussion that didnt target any specific changes.
Continue a quick discussion about the possible topics every teacher would like to get
some feedback about.
Teaching: Describe how the strategy/concept/skill will be introduced/modeled. (1-3 mins)
First, I will show the student a completed survey on Survey Monkey. A quick look at the
summary tab and analysis tab will help the student see the information provided by
implementing this kind of survey. Next, I will show the student the steps to creating a
survey by first starting at create survey naming the survey pick Education from
drop down box Lets Go. On the create page I will explain the different options on the
left hand side to use to create a personalized survey. I will explain that she can add her
own questions by clicking add a question and choosing an option from the drop down
box. Finally click Done.

Active Involvement: Describe what students will do in a short practice session to demonstrate
understanding. (5-8 mins)

The student will create their own survey using Survey Monkey. First, the student needs
to identify a topic for the survey. Following my directions, the student will design their
own survey. When the survey is completed the student will email it the intended people.
Closure: Describe how you will provide an opportunity for students to share work, ideas,
review, etc. (1-2 mins)
Once the survey is completed, the student will give a brief verbal summary of how easy
or difficult the survey was to create and if the survey gave the teacher insight to the

To whom did you teach the lesson?


I taught this lesson to a friend of mine who is not familiar with Survey Monkey. She is a
childrens ministry coordinator and has constant contact with parents and children. This
type of survey could greatly benefit her work by providing a quick, convenient way of
surveying all parties involved.

How did you ensure active participation?


I was there for her to ask questions and she emailed the survey to me. Since it was a oneon-one lesson it was easy to help her through the process.

How did you check for understanding?


She explained her rationale for which questions she chose. She also was able to talk about
how to use the different options when creating the survey.

Reflect upon what went well.


The anticipatory set helped engaged the student to learn the new technology. She
immediately could think of questions and topics worth surveying parents and students
about. Showing the steps all the way through helped give the students a good overview of
how the website was set up and options available.

Reflect upon the challenges and describe how you addressed the challenges.

A challenge was narrowing in on one topic. It was difficult for the student to stay focused
when picking and creating questions. Its important to stay focused so that the results will
give insight on the topic.

What would you do differently to make the lesson more effective in the future?
Next time I would make a quick hand out on the steps. Also, it would be beneficial to
review tips for picking questions to focus on one topic at a time.
