PMC-FM-HSS-024 Near Miss Report

A contractor reported a near miss incident on a project where the foreman noticed white smoke coming from the engine of the pickup he was driving. Upon inspecting the engine, it was discovered that the smoke was caused by overheating. No one was hurt in the incident. As a corrective action, the contractor provided a flagman to assist operators and gave a safety talk to operators about safely operating equipment.

While driving along D 42 Mr. Ravi, foreman, noticed white smoke coming from the bonnet. He immediately pulled
over, turn off the pickup. After opening the bonnet and discovered the smoke is due to an overheating engine.
Nobody gets hurt on what happened.

Provided flagman to assist the operator in operating heavy equipment, for safe working in excavation. Given tool
box talk to the Operator.
