Branch CPC
The securities are available for demat --
Whether face value.ofthe securities available for demat
The scrip is not listed in the defaulter/banned companies provided by

the Depository
Name in the DRF and certificate should match
Total number of securities as per certificate and DRF tallies
Quantity of securities in words and figures correct
The certificate number, folio number and distinctive numbers
mentioned in the DRF tallies with the certificate
Signature of the client(s) on the DRF tallies as per the specimen
recorded with us
Client has filled in separate DRF for securities having distinct ISINs
Client has filled in separate DRF for locked in and free securities having

Acknowledgement slip duly signed, stamped and returned to client . 1------

Branch inward stamp, date, name and signature of DP official available

Cancel the certificate by drawing two parallel lines across the certificate
and punch two holes on the company name
In case of death of one of the holders, notary attested copy of the death
