Interview Release Agreement

Interview Release Agreement

Grant

_____________ ("Author") has informed me that she is researching, writing, and publishing a
one or more books and related works on the subject of _______ (the "Work"). In order to assist
Author in preparation of the Work, I, _______________, consent to the recording of my
statements and grant to Author and Author's assigns, licensees and successors 1) the right to
copy, reproduce, and use all or a portion of the statements (the "Interview") for incorporation in
the Work and 2) the right to copy, publish, display and use any photographs and documents (the
"Materials") that I may choose to give to Author.

For consideration which I acknowledge, I permit the use of all or a portion of the Interview and
the Materials in the Work in all forms and media, including advertising and related promotion, in
perpetuity and throughout the world. I grant Author the right to use my name, image, likeness,
and biographical data in connection with all uses of the Interview. I waive the right to inspect or
approve use of my Interview as incorporated in the Work.

Release

I release Author and Author's assigns, licensees and successors from any claims that may arise
regarding the use of the Interview including any claims of defamation, invasion of privacy, or
infringement of moral rights, rights of publicity or copyright.

I acknowledge that I have no ownership rights in the Work.

I acknowledge and agree that I am not entitled to receive any form of payment from Author and
Author's assigns, licensees and successors.

I have read and understood this agreement and I am over the age of 18. This Agreement
expresses the complete understanding of the parties.
