QUESTIONAIRES FORM

PURPOSE
The survey is consists of 15 questions and should take approximately 2-5 minutes to complete. All information given will be
CONFIDENTIAL and purposely for research and reference academic only.
The survey is intended to provide this Team with valuable feedback to ensure that this company is focusing the efforts in the
right areas that will deliver a defined improvement for employees; with this in mind can I ask that everyone participates in
order to achieve the goal of continuous improvement/raising the standards.
After the completion of the survey a comprehensive analysis of the data provided will be conducted, which should take just a
few weeks. Therefore, we expect to be able to share the initial results with you and discuss what we aim to do with the
