Use of form: Completion of this form is required by HFS 45.06(8)(d)2, HFS 46.08 (7)(b) and HFS 52.47 (6)(a)1. Failure to complete
may result in a licensing enforcement action. Personally identifiable information on this form will be used only to verify the vehicle
inspection.

Instructions: This form is to be provided to a garage, dealership or auto repair shop, to be completed by them after inspecting the
vehicle. The licensee shall return the completed form to the Licensing Specialist.
