GUIDELINES FOR THE PDA COLGATE FREE DENTAL

PROJECT NAME:

ZERO CAVITY MISSION-KABARANGAY SA DENTAL HEALTH

OBJECTIVES:

Promote oral health awareness among Filipinos


Determine the DMFT of the residents of the barangays
Recommendation and referrals for treatment

TARGET PATIENTS

Maximum of 500 patients of all ages per barangay

SERVICE PROVIDERS

Colgate will provide dental charts, VCD for oral health lectures, posters,
streamers,
Local PDA Chapters will provide oral health education, free dental check-up
Barangay officials will provide audio-visual equipment if they have.

MECHANICS

1. Choose a barangay preferably near the headquarters of your chapter to


minimize travel time. The barangay should be easy to locate for the Colgate
merchandisers and PDA-Colgate Project Coordinator.
2. Make the necessary coordination with the officials of your chosen barangay.
3. Provide an area in the barangay where the chapter’s volunteer dentists can
conduct oral health lectures, video presentation and free dental check up.
4. Free dental check up will be rendered among 500 patients. Colgate will
provide dental charts and waivers
5. Registration will be handled by the dental chapter with the help of the
barangay officials and barangay health workers.
6. PDA Local Chapters will conduct oral health education among residents of
the barangay using flip charts, videos and tooth models.
7. Colgate samples will be provided for a maximum of 500 patients per
barangay.
8. Search for Zero Cavity Kid for kids 6 to 12 years old. Colgate Gift pack will
be given to winner.
9. The PDA Local Chapter can render other dental services provided they have
the necessary dental materials needed for the service be it preventive or
curative services.
10. Cash subsidy of Php 6,000.00 will be given to the chapter for to cover their
transportation, food and other expenses. The subsidy will be given by
Colgate Merchandisers on the day of chapter’s visit to the barangay.
11. Colgate T-shirts and gift packs will be given to 10 volunteer dentists from
the local chapter.
12. Colgate products will be given to barangay officials.
13. The Chapter can provide other dental services they can offer provided they
have the necessary materials needed for the treatment.
