Signature must be that of the parent or legal guardian of the child. All legal guardians
and/or parents must be contacted prior to sacraments being conferred.

To process your request, please return this forms with a copy of your child’s Baptismal
Certificate and a $25.00 “certificate fee” to the Faith Formation Office, room 258, (2nd floor)
Wednesday thru Friday, 9:00 AM to 5:00 PM, Saturday from 9:00 AM to 4:00 PM.

Both parent and child will be required to attend specific Parent Sacramental Process classes
in preparation for this wonderful event. The schedule will be published on our website, is
available at the Faith Formation Office, or by contacting the program coordinator at the
number/email listed below. MSLRP Faith Formation Department has the responsibility for
processing sacramental documents for our parish school. All certificates will be mailed to
