Dear Governor Mendoza:

This is to reiterate our previous request dated March 23, 2011 for the hiring of three (4) Job Orders to
be charged against the funds of this office – Other Professional Services - General Fund Proper - Account
799 – 1 and appropriated in the Budget Year 2011, as follows:

1. One Job Order (Nutritionist) to be assigned in the Nutrition Program. Ms. Lutgarda B. Perocho had
retired from the service effective April 1, 2011. I strongly recommend Ms. Lene Lydia E. Sirilan a
former Nutritionist Casual and was not renewed up to the present to assist Ms. Jasmine C.
Regaspi – Administrative Assistant 111 – Nutritionist who will be assigned to take charge of the
nutrition program of the province.

2. One Job Order (IPHO health programs – Administrative Aide as Clerk) – to replace Ms. Jasmine C.
Regaspi – Administrative Assistant 111 assigned in the nutrition program at the same time act as
Storekeeper.

3. One Job Order (Administrative Aide) preferably male and computer literate to be assigned in the
office of the undersigned to assist the PHO in all his activities.

4. One Job Order (Administrative Aide - Utility Worker, preferably female) to do the day to day
activities in cleaning the entire IPHO building and its surroundings. Recommending Ms. Cherry C.
Montalla, a former casual of the IPHO.

I am looking forward that this request be given a favorable consideration and approval.
