Problem

Goals and objectives LTO: Within 2 days of nursing intervention, the client will be able to Void in normal amounts of larger than or next to to thirty mL/hour, as well as common pattern. STO: after 10 hrs of nursing intervention, the client will be able to:  Enumerate the importance of increasing fluid intake  Increase urinary output gradually  Demonstrate techniques to prevent urinary infection  Verbalize understanding of condition

Interventions > Monitor I&O and characteristics of urine. >Detrmine patients normal voiding pattern and note variations > Observe for changes in mental status, behavior, or level of consciousness > Administer medications as indicated >noted reports of exacerbations and spontaneous remissions of sx of urgency and frequency >assisted with developing toileting routines >discussed possible diet restrictions >Encouraged clirnt to verbalize fears and concerns

Evaluation Goal met if :the client will be able to:  Enumerate the importance of increasing fluid intake  Increase urinary output gradually  Demonstrate techniques to prevent urinary infection  Verbalize understanding of condition Goal partially met if: the client will be able to do one or two objectives within 10 hours. Goal not met if: the client will not be able to do any of these activities.
