Waiver Highway Engineering

This waiver form from Adamson University's Civil Engineering Department allows a student to conduct highway engineering surveys outside of the university but requires the student's parent or guardian to release the university from any legal claims arising from injuries during the surveys. The parent must sign giving their permission for the student to participate in the off-campus activity and for any charges to be billed to the student's account.
