Behavior Reflection Sheet

A student behavior reflection sheet asks the student to identify which class rules they failed to follow that day by circling the applicable rules. It then prompts the student to explain what they could have done differently to prevent getting in trouble and to outline their plan to improve their behavior the next day. The form is to be signed by a parent or guardian and allows space for teacher comments.
