Gate Pass Request Document

Bureau Veritas (India) Pvt. Limited requests an extension of staff and vehicle gate passes from the Vice President of Projects at BALCO-III Smelter Expansion Project until September 30, 2012. The current gate passes are set to expire on June 30, 2012 but Bureau Veritas' work order has not been extended yet. Bureau Veritas also provides copies of medical checkup reports and safety induction formats for staff and drivers to BALCO for their records. Finally, Bureau Veritas requests permission to prepare a new replacement gate pass for an office printer that was brought through the SEPCO Gate as the original gate pass was misplaced.

We are delivering third Party services at BALCO Smelter Expansion Project vide LOI / Work Order No. Agreement 6600000121 dated 14/05/09, which was expired on 30/03/2012. As our entry pass are renewed up to 30/06/2012 with your permission and as on date we have not received our work order extension and keeping progress of work in view we request you to extend the validity of the gate passes of staff, driver and vehicles accordingly, preferably 30th September 2012

We are delivering third Party services at BALCO Smelter Expansion Project vide LOI / Work Order No. Agreement 6600000121 dated 14/05/09, for our office work we have brought a HP laser jet Scanner and Printer, bearing serial number CNG895NOBW through SEPCO Gate.. As the original gate pass of this printer was misplaced, we request your permission to prepare NRGP based on Xerox copy of this Gate pass Your cooperation for taking out this printer for maintenance is requested
