Very Important: Students Must Read the Following Instructions Carefully and Act Accordingly
1. Completion of all graduation requirements is each students own responsibility. Please fill in the Graduation Application Form Properly and attach the following documents (attested by the 1st class gazetted officer/a senior faculty member of UIU/Notary Public) with the Graduation Application Form before submitting it to the Respective Department/ Program Office: For Undergraduate Programs a. Four Copies of Recent Passport Size Photographs in formal dress in a separate Envelope. b. Certificates of SSC & HSC / O & A Level/ equivalent (as the case may be). c. Mark Certificates / Transcripts of SSC & HSC / O & A Level/ equivalent (as applicable). d. Testimonials (if any) of SSC & HSC / O & A Level/ equivalent (as the case may be). e. A photocopy of the Money receipt of Graduation Fee amounting Tk. 4,000 (four thousand). For Graduate Programs a. Four Copies of Recent Passport Size Photographs in formal dress in a separate Envelope. b. Certificates of SSC & HSC / O & A Level/ equivalent, Bachelors / Masters Degree (as applicable) c. Students have to deposit the Graduation Fees of Tk. 4,000 (four thousand) by filling up the Deposit Slips of Mutual Trust Bank, Dhanmondi Branch, Dhaka available from UIU Registrars Office against A/C # 0014-0310002818. Students must mention Convocation 2010 on the deposit slips. Submit the completed Graduation Application Form to the Registrars Office by April 15, 2010 for reviewing the completion of degree requirements and other formalities. 3.
