Description of Setting The patient was fetched at the Rehab male Ward who appeared clean and calm. Then after, the resident was brought at the Social Hall where the interaction and execution of the planned activities for the day took place. For the exercise, we have place them outside the hall for a fresh air because it was surrounded by trees and is conducive for therapy and then inside the social hall for NPI. Chairs were provided for the patient to have a comfortable stay. The environment during the nurse-client interaction was made therapeutic by sitting across the client and assuming an L position. General Objective The student nurse aims to maintain the rapport with the client and continue to use therapeutic communication skills to be able the client to verbalize her feelings.
Specific Objectives The specific objectives of the student nurse are the following: To be maintain trust rapport with the resident To further improve therapeutic communication skills and practice active listening more effectively To assist in performing the activities of daily living while promoting independence of the client To be able to explore what the client feels or thinks To maintain patients safety and be free from harm at all times The specific objectives for the client are as follows: To be able to perform ADLs with assistance To participate in the planned activities for the day To be able to express her thoughts and feelings throughout the interaction and during the implementation of therap

It seems that the patient still appreciated the nurse even though he has mood changes all throughout the NPI.
Summary: Today has been more successful than yesterday! The patient was cooperative though there are times that his mood changes not like yesterday that he was irritable for a long period of time. Nurse patient interaction today is a little bit okay. I have taken some information about him that I think needed to be more elaborated tomorrow. The patient enjoyed our play and dance therapies that made me knew that she is fond of dancing and also loves music. Good communication technique and patience is still the best factor in dealing with this kind of patient. However, I know that my skills should improve more to further understand the patients thought process and cognition.
