The vSCAT screen is divided into two tabs. There is a General tab for administrative entries and incident descriptions and an Analysis tab to run the cause analysis. Tip: While it is not required, we do suggest you attend the DNV course titled Incident Investigation offered by Burnell & Associates Inc. General Tab
The General Tab collects the standard data required for the administrative details of the Incident Investigation. If vSCAT is being used in addition to another incident investigation system, the General tab can become the reference page or title page for the causal analysis section of the investigation.

Case Name A unique title or name (max 25 characters) for this Incident. Loss Severity Potential The level of potential loss of this Incident. Date Occurred The date the Incident occurred. Frequency of Exposure The regularity of this occurrence . Probability of Recurrence- The potential for the incident to recur. Incident Description Full explanation of the incident. General Help - Help with actions and definitions within the Tab . Field Help Help definitions for data entry within each field.

Analysis Tab
The Analysis tab is divided up into four sections. Each of these sections contains a list of options that can be selected to describe each stage of the Incident. These lists are hierarchical in nature and each entry in a given section directly relates to the entry in the section above it.

Type of Event: Incident/Contact- Select the type of Incident or type of Contact that ocurred. Immediate/Direct Causes- Select the Immediate/Direct cause related to the Incident or Contact. Basic/Root Causes Select the Basic/Root Causes related to the Immediate/Direct Causes . Corrective Actions Required- Select the Corrective Actions related to the Basic/Root Causes. General Help - Help with actions and definitions within the Tab (Form) . Field Help Help definitions for data entry within each field. Add Button- Add a new selection from the lists. Edit Button- Edit the comments or change an existing selection. Delete Button- Delete a selection or comments.

Select File > New > General Tab and enter all of the required information in the General Tab

Add New
To add a new Type of Event: Incident/Contact , click on the Add button to the right of the window.
This will bring up the New Type of Event: Incident/Contact window ready for data entry.

Edit Existing
Select the desired record from the list by clicking anywhere on the desired row. The row will turn blue to indicate your selection. Click on the Edit button to the right of the list.

Make your desired changes and click on the OK button to save your work. Click on the Cancel button to abort the operation and close the window. If you clicked on the OK button, the detail screen will close and the information will be displayed in the selected row in the first list in the Cause Analysis screen.

If it is safe to delete the record, you will receive the warning message shown above. If you click on the Yes button the record will be deleted. Clicking on the No button will abort the operation. If the record you have selected is not safe to delete (it has associated Immediate/Direct Causes), you will get this error message:

Add New
As there can be more than one Causal Analysis String, before adding a new selection from List 2, you must first highlight a record in List 1 (Type of Event) to establish the parent-child hierarchical relationship. Select the desired record from List 1 by clicking anywhere on the desired row. The row will turn blue to indicate your selection.
Now you can click on the Add button to the right of List 2. This will bring up the new Immediate/Direct Cause(s) screen ready for selection and data entry.

Note: In any Investigation there is always more than one reason for an incident. Each Causal Analysis string will define an individual action or condition that contibuted to the incident. In this Causal Analysis String, the list for Substandard Conditions has been selected as the investigation has determined that one of the causes was defective materials .
Note: The title of this Analysis String , selected in List 1, will be displayed in the screen banner at the top of the form. This will be a reminder of which Causal Analysis String you are working in.

There is an additional filter to the right of the drop-down menu, called Show Only Probable.

Click on the Delete button to the right and the following warning window will appear.
If you click on the Yes button the record will be deleted. Clicking on the No button will abort the operation. If the record you have clicked on is not safe to delete (it has associated Basic/Root Causes), you will get this error message:

As there are multiple causes/reasons for every Incident, there will be multiple Causal Analysis Strings to define each of those causes. Before adding a new selection from List 3, highlight the related selection in List 2 to establish the parent-child hierarchical relationship. Highlight the related selection from List 2 by clicking anywhere on the desired row. The row will turn blue to indicate your selection.

1. Factor Drop-Down Box-Select either Job Factor or Personal Factors. 2. Basic/Root Cause(s) The Category List which loads based on what is selected in the Factor Type drop-down menu. 3. Basic/Root Cause(s) Sub Category The Sub-Category list which is loaded based on what is selected in the Category list. 4. Explanatory Note - Enter details on selections 5. SCAT Definitions A definition for each cause. 6. General Form Help Guidance on using Form 7. The title of the record you selected in List 3 will be displayed in the window banner at the top. There are two help fields at the bottom of this window. Through statistical analysis it can be predicted, with a high degree of probability, which records will be needed as a result of the parent record selection. Checking this box will cause the left list to only reveal these probable records. This screen is divided into two sections, Category and Sub-Category, which allow you to further define and clarify this level in the hierarchy. Select the most appropriate entry from the list. This will generate a list of entries in the SubCategory list. The Sub-Category list is optional but does help to further define this level. Enter any applicable notes in the field on the right. To save your work and close the screen, click on the OK button. Clicking on the Cancel button will abort the operation and close the screen.
