vSCAT - Systematic Cause Analysis Technique (SCAT) Software

This software version of SCAT allows you to use the causal analysis methodology that is the commonly accepted standard for Incident Root Cause Analysis. Many safety and risk management organizations have adopted this standard worldwide. vSCAT uses the standard created by Det Norske Veritas called Systematic Cause Analysis Technique or SCAT. The Incident report is created and defined through the General tab and analysed, along with the SCAT Charts, through the Analysis tab. The SCAT Definitions and Help menu guide you through the process from the Incident/Contact, Immediate/Direct Causes (including Substandard Acts & Substandard Conditions), Basic/Root Causes(Including Job Factors and Personal Factors) to Corrective Actions. Comments can be included at each level defining the details of each specific factor. As there is always more than one cause for any Incident, you will be able to print a report defining each Causal Analysis String including Event , Direct Cause, Root Cause and subsequent Corrective Action related to the event you are Investigating.

The vSCAT screen is divided into two tabs. There is a General tab for administrative entries and incident descriptions and an Analysis tab to run the cause analysis. Tip: While it is not required, we do suggest you attend the DNV course titled Incident Investigation offered by Burnell & Associates Inc. General Tab
The General Tab collects the standard data required for the administrative details of the Incident Investigation. If vSCAT is being used in addition to another incident investigation system, the General tab can become the reference page or title page for the causal analysis section of the investigation.

Analysis Tab
The Analysis tab is divided up into four sections. Each of these sections contains a list of options that can be selected to describe each stage of the Incident. These lists are hierarchical in nature and each entry in a given section directly relates to the entry in the section above it.

Type of Event: Incident/Contact- Select the type of Incident or type of Contact that ocurred. Immediate/Direct Causes- Select the Immediate/Direct cause related to the Incident or Contact. Creating a Causal Analysis Report using vSCAT

The Causal Analysis Report starts with collecting the information about the incident. A factual accounting of the event along with designating the levels of the selected criteria ( Loss Severity Potential, Frequency of Exposure and Probability of Recurrence) , will slot the incident at the proper investigation level. For example, selecting Major for Potential Loss Severity (potential injury or loss of life, environmental damage, loss of production or loss of property) should slot this investigation at a High Priority level. Each of these three functions can help determine the priority of the investigation and the level of expediency at which the corrective actions should be implemented.

Select File > New > General Tab and enter all of the required information in the General Tab

Analysis Tab List 1 Type of Event: Incident/Contact

The Causal Analysis String starts with the Type of Event: Incident/Contact List. There can be many Causal Analysis Strings for each Incident, each with its own Corrective Action, as there is always more than one factor responsible an Incident.

Add New
To add a new Type of Event: Incident/Contact , click on the Add button to the right of the window.

This will bring up the New Type of Event: Incident/Contact window ready for data entry.

Select the most appropriate Type of Event entry from the list and enter any applicable notes in the Explanatory Notes field. Note: There are two help fields at the bottom of this window. The field at the bottom of the form contains Help for the entire window or form and the SCAT Definitions field contains Help for the item selected in the list. To save your work and close the screen, click on the OK button. Clicking on the Cancel button will abort the operation and close the screen.

Note: In any Investigation there is always more than one reason for an incident. Each Causal Analysis string will define an action or condition that contibuted to the incident. When the OK button is clicked, the detail window will close and the information will be displayed in the next available row in the first list in the Cause Analysis window.

Edit Existing
Select the desired record from the list by clicking anywhere on the desired row. The row will turn blue to indicate your selection. Click on the Edit button to the right of the list.

The Edit Type of Event: Incident/Contact window will be displayed, containing the record data. The Type of Event and the Explanatory Note can be changed.

Make your desired changes and click on the OK button to save your work. Click on the Cancel button to abort the operation and close the window. If you clicked on the OK button, the detail screen will close and the information will be displayed in the selected row in the first list in the Cause Analysis screen.

Select the desired record from the list by clicking anywhere on the desired row. The row will turn blue to indicate your selection. Click on the Delete button to the right of the list. Note: Because of the hierarchical nature of these lists, you cannot delete an entry unless all related entries in the lower lists are deleted first.

If it is safe to delete the record, you will receive the warning message shown above. If you click on the Yes button the record will be deleted. Clicking on the No button will abort the operation. If the record you have selected is not safe to delete (it has associated Immediate/Direct Causes), you will get this error message:

List 2 Immediate/Direct Cause(s)

Immediate/Direct Cause(s) are the symptoms ( behaviours , circumstances and deviations from a standard practice ) directly related to the Incident and is the next determination within each Causal Analysis String. The selections are filtered under two headings: Substandard Acts and Substandard Conditions. Each of these two Headings have unique selection lists which, when included in the Causal Analysis String, will help define a causal factor and produce a unique Corrective Action.

Add New
As there can be more than one Causal Analysis String, before adding a new selection from List 2, you must first highlight a record in List 1 (Type of Event) to establish the parent-child hierarchical relationship. Select the desired record from List 1 by clicking anywhere on the desired row. The row will turn blue to indicate your selection.

Now you can click on the Add button to the right of List 2. This will bring up the new Immediate/Direct Cause(s) screen ready for selection and data entry.

Within the Immediate/Direct Cause(s) list (List 2) there is an option to choose whether this Immediate/Direct Cause was a Substandard Act or a Substandard Condition . From your Investigation notes , determine which direction this string will take.

Note: In any Investigation there is always more than one reason for an incident. Each Causal Analysis string will define an individual action or condition that contibuted to the incident. In this Causal Analysis String, the list for Substandard Conditions has been selected as the investigation has determined that one of the causes was defective materials .

Note: The title of this Analysis String , selected in List 1, will be displayed in the screen banner at the top of the form. This will be a reminder of which Causal Analysis String you are working in.

Filter the selection list by choosing an entry from the drop-down menu at the top of the screen. By selecting Substandard Conditions, the list is filtered to show the Substandard Conditions only. When Substandard Acts is selected, a new list with only Substandard Acts is revealed.

There is an additional filter to the right of the drop-down menu, called Show Only Probable.

Through statistical analysis, it can be predicted(with a high degree of probability), which records will be needed as a result of the parent Type of Event selection. Checking this box will filter the list to reveal those probable records only. Select the most appropriate Substandard Condition entry from the list. Enter any applicable notes in the Explanatory Note field. To save your work and close the screen, click on the OK button. Clicking on the Cancel button will abort the operation and close the screen. If you clicked on the OK button, the detail screen will close and the information will be displayed in the next available row of the second list on the Cause Analysis screen.

Edit Existing
Highlight the desired record from the list by clicking anywhere on the desired row. The row will turn blue to indicate your selection.

Because of the hierarchical nature of the Individual Causal Analysis Strings, the entry cannot be deleted unless all related entries in the string beneath it are deleted first. Once the related factors beneath this Selection have been deleted, highlight the selection by clicking anywhere in the desired row. The row will turn blue to indicate your selection.

Click on the Delete button to the right and the following warning window will appear.

If you click on the Yes button the record will be deleted. Clicking on the No button will abort the operation. If the record you have clicked on is not safe to delete (it has associated Basic/Root Causes), you will get this error message:

List 3- Basic/Root Cause(s)

Basic/Root Cause(s )are the underlying factors or real reasons why Substandard Acts and Conditions occur and is the next related determination within each Causal Analysis String. The selections are filtered under two headings: Job Factors and Personal Factors. Each of these two Headings has a unique list of factors, that when selected, opens a sub-category to further define the factor selection in the Causal Analysis String.

Add New
As there are multiple causes/reasons for every Incident, there will be multiple Causal Analysis Strings to define each of those causes. Before adding a new selection from List 3, highlight the related selection in List 2 to establish the parent-child hierarchical relationship. Highlight the related selection from List 2 by clicking anywhere on the desired row. The row will turn blue to indicate your selection.

Now click on the Add button to the right of List 3. This will bring up the New Basic/Root Cause(s) window ready for data entry. Note: The title of the related selection from Immediate/Direct Causes (List 2) will populate the banner.

Select the desired record from the list by clicking anywhere on the desired row. The row will turn blue to indicate your selection.

List 4 Corrective Actions Required

Add New
Before adding a new record in this list, you must first select a record in List 3 (Basic/Root Causes(s) to establish the parent-child hierarchical relationship. Highlight the re record from List 3 (Basic/Root Causes(s)) by clicking anywhere on the desired row. The row will turn blue to indicate your selection.

Click on the Add button to the right of List 4 (Corrective Actions Required). This will bring up the New Corrective Actions Required window ready for data entry.

The title of the record you selected in List 3 will be displayed in the window banner at the top. There are two help fields at the bottom of this window. There is a check box above the Corrective Actions Required-Category field called Show Only Probable. Check to show only probable selections.

Through statistical analysis it can be predicted, with a high degree of probability, which records will be needed as a result of the parent record selection. Checking this box will cause the left list to only reveal these probable records. This screen is divided into two sections, Category and Sub-Category, which allow you to further define and clarify this level in the hierarchy. Select the most appropriate entry from the list. This will generate a list of entries in the SubCategory list. The Sub-Category list is optional but does help to further define this level. Enter any applicable notes in the field on the right. To save your work and close the screen, click on the OK button. Clicking on the Cancel button will abort the operation and close the screen.

If you clicked on the OK button, the detail screen will close and the information will be displayed in the next available Corrective Actions Required row. The full Cauasal Analysis String is now complete and highlighted in blue.

Make your desired changes and click on the OK button to save your work. vSCAT Delete Existing
As this list is the lowest in the hierarchy, there is no need to check for dependencies before deleting. Select the desired record from the list by clicking anywhere on the desired row. The row will turn blue to indicate your selection.

Click on the Delete button to the right of the list. You will receive this warning message:

If you click on the Yes button the record will be deleted. Clicking on the No button will abort the operation.

Defining the individual Cause Analysis Strings

You can enter as many Causal Analysis Strings as are required. When the start of the Causal Analysis String is selected and highlighted in Type of Event, the balance of the Causal Analysis String will appear. (Example: Abnormal Operation is the start of Causal Analysis String 1 and when it is selected the related Immediate/Direct Causes, Basic/Root Causes and Corrective Actions come into view.) Note: Only one Causal Analysis String can be viewed at one time.

Note: The hierarchical set up of the Causal Analysis Strings with Type of Event, Immediate/Direct Causes, Basic/Root Causes and Corrective Actions listed in that order. Since vSCAT is meant to compliment the DNV Scat Chart, only a printed copy of the vSCAT Analysis Summary is available.
