Instructions for Self-made Med Cards: Med cards must be handwritten on a 5x8 Index Card; use both sides of card. Use sample as a guide. Maintain Med Cards in an organized fashion for easy retrieval. All med cards must be legible. The following format must be used for completion of each drug card:
Classification-highlight in yellow Generic & Trade Names-each card can list up to four meds within a classification Routes Dosages IV Compatibility Therapeutic Effect- Mechanism of action & indications; Why med ordered? Interactions with other drugs, OTC, or herbal medicines Nursing Implications: Contraindications/warnings/interactions Common side effects- Capitals indicate life threatening; underline indicates most frequent Lab value alterations Patient teaching Pre-administration assessment Why hold or not give med Evaluation
