Thank you for your interest in Head Start School System, our Registration Forms for admission are
available online and can be downloaded to be filled out manually, or can be submitted online through
our website as per your convenience.

January 2013 (9:30 a.m. and 4:30 p.m.)

Requirements:
Please bring along the following for registration purposes in both cases, whether downloaded forms or forms
submitted online:
1. Birth Certificate (those made in the last six months will not be accepted).
Please bring along the original birth certificate (this will be returned)
You are required to submit a photocopy of the same.
2. Four passport sized photographs
3. Progress report of the previous school (if any).
4. A printout of the form submitted online.
For Registration of students for Reception, Pre Nursery and Adv. Nursery.
Both parents along with the child are requested to come for the Registration, an interview will be conducted
on the spot.

PLEASE FOLLOW THE STEPS BELOW:
A. Downloaded Forms:
1. Please take a printout on an A4 sized sheet and fill in all details by hand in block letters (all areas are
mandatory).
2. Please bring the form to the concerned branch where you wish your child to be registered, as per the
Registration Schedule
3. Submit the form along with a registration fee of Rs. 500/- (please collect the receipt).
4. Receive a slip for test date/time
5. If an interview is required you will be informed subsequently.

B. Forms Submitted Online:
1. Please fill in all the fields as they are all mandatory.
2. Click to submit
3. You will receive an email informing you about your registration ID and confirming all the information you
entered online, therefore please be careful to enter your valid and current email address on the form.
4. Kindly print your confirmation email and bring it with you when you visit the concerned branch where you
wish your child to be registered, as per the Registration Schedule.
5. Submit the form along with a registration fee of Rs. 500/- (please collect the receipt).
6. Receive a slip for test date/time
7. If an interview is required you will be informed subsequently.
