Lacebal, Male, 25 years old, came for consultation and admission on December 7, 2013 for infectious tropical disease and severe fever resulting in bleeding, low levels of blood platelets and blood plasma leakage. He also consulted for the numbness of his left leg to lower foot. He was seen and examined for the following findings: 1. Recurrent Cellulitis, Left foot to lower leg 2. Dengue Hemorrhagic Fever

Operation/Special Procedure (if applicable): n/a Recommendation: * Confinement, blood transfusion and Bed Rest and fluid intake is absolutely necessary for the restoration of his health

Remarks: The patient is now in normal condition as per January 13, 2014. This certification is being issued upon request of the parents of the patient for whatever purposes he may deem proper, except for medico legal purposes.
